Income and Expense Survey Forms:

Pursuant to the authority provided in the Code of Virginia § 58.1-3294, the City of Fairfax Real Estate Assessment Office collects and analyzes information for the annual reassessment of real property. Therefore, the city  requests owners of income-producing real property in the City of Fairfax submit the Income and Expense Survey attributable to the property over the previous July to June of the current year for each such parcel of real estate. Income and Expense Survey information submitted will be kept confidential in accordance with Virginia Code § 58.1-3. 

All Income and Expense Surveys must be certified with the signature of the owner, an officer of the corporation, or the duly authorized agent of the owner. If the survey is certified by an agent, a letter of authorization (LOA) from the owner to do so must be submitted with the statement. LOA’s must be complete, notarized, bearing original “wet” signatures.

The survey forms below are available for download to print or  fillable which may be emailed to RealEstate@fairfaxva.gov.  Income and Expense information must be entered on a survey form retaining the same format.  Accompanying documents such as rent rolls, income statements, and receipts for capital expenses paid may be attached.
